AN ORDINANCE
AMENDING TITLE 16 OF
THE JOHNSON COUNTY
CODE
OF ORDINANCES FOR
FEES

WHEREAS, the John-
son County Health De-
partment has the authority
to charge a fee for certain
services pursuant to Indi-
ana Code 16-20-1-27;
WHEREAS, the John-
son County Health De-
partment seeks to clarify
and streamline its fees for
services and the require-
ments for mobile food,
temporary, and organized
event permits;
WHEREAS, based on
these revisions, it is nec-
essary for Johnson
County to update its rules
regarding fees as set forth
in the Johnson County
Code of Ordinances.
NOW, THEREFORE,
BE IT ORDAINED as fol-
lows:
Johnson County Code
Title 16, Public Health,
shall be amended to con-
tain the following:
16-1-1-2 FEES FOR
SERVICES.
(A)The Board of Health
has authority to charge a
fee for certain services.
Those fees shall be as
follows:

On-Site Septic

stallation of new on-site

The Johnson County
Health Department must
be contacted prior to any
changes to a new septic
system installation permit,
a septic system repair per-
mit, or a holding tank in-
stallation permit. Any
change to said permits
may require a new appli-
cation and permit, includ-
ing fees, at the discretion
of the Johnson County
Health Department.

*A renewal invoice for
yearly licensed food facili-
ties will be mailed in late
October. If the renewal is
not post marked by De-
cember 31, a double li-
cense fee will be charged
to the facility.
***Event organizer must
notify the Johnson County
Health Department at
least 30 days in advance
of the event. A vendor will
not be granted a permit if
the organizer fails to notify
within 30 days. **A re-
newal invoice for yearly li-
censed facilities will be
mailed in late October. If
the renewal is not post
marked by December 31st
then a double license fee
will be charged to the facil-
ity.

Nursing (effective July
28, 2015)
The final cost of pur-
chased vaccine will be de-
termined by the cost of the
vaccine, taxes, shipping,
handling, processing, and
administrative fees and
will be billed to the pa-
tients contracted insur-
ance company.
If the patient chooses to
pay for the vaccine, the fi-
nal cost will be the vaccine
cost per dose rounded up
to the nearest five-dollar
($5.00) increment plus an
administrative fee.
TB Skin Tests Cost of

pay the administration fee.
(B) Food Establishment
licenses to operate are not
transferable from one
person/owner to another.
A new license to operate
is required whenever there
is a new person/owner
and the new license shall
be obtained prior to the
conduct of business under
the new owner/person.
(C) All food establish-
ment licenses, pool per-
mits, tattoo/piercing/body
modification facility per-
mits and septic permits
are subject to a double fee
if any activity which re-
quires a license/permit
takes place before the
necessary
licenses/permits are ob-
tained.
(D)Returned Check
Fee: $20.00 plus an
amount equal to the actual
charge by the depository
institution for each re-
turned or dishonored in-
strument, pursuant to
IC 26-1-3.1-502.5
16-1-1-3 REQUIRE-
MENTS FOR MOBILE
FOOD ESTABLISHMENT,
TEMPORARY, AND
ORGANIZED EVENT
HOLDERS
(A) Operation of a Mo-
bile Food Establishment,
even if such operation
only occurs seasonally, re-
quires an operator to ob-
tain a Mobile Food Estab-
lishment permit or tempo-
rary permit. In no event
shall a Mobile Food Estab-
lishment obtain an organ-
ized event permit.
(B) Operators of a Mo-
bile Food Establishment
and organized event per-
mit holders shall physically
return to an approved
commissary at least once
daily for any support activ-
ities, as defined in 410
IAC 7-26, et seq. Permit
holders must submit a
commissary kitchen
agreement for review and
approval by the Author-
ized Representative.
